So… What is Sciatica, really?
Sciatica refers to irritation of the sciatic nerve or its nerve roots as they leave the lower spine. It can be triggered by a disc herniation or bulge, joint restrictions, narrowing around nerve exit points, inflammation and/or soft tissue tension that alters load and movement. This is why two people with the same MRI scan finding can feel completely different. Or, why two people with the same symptoms can have completely different MRI scans!! How Confusing!!!
Some people have significant pain with minimal imaging findings. Others have disc changes on MRI with no symptoms at all. That is why a scan alone is not the full story. What matters most is how your spine is functioning and how your nervous system is responding. Function over feeling is VITAL.
Why timing matters: early sciatica treatment is not only about pain relief
Most people wait. They rest, stretch, take anti-inflammatories or try to push through. Sometimes that helps. Often it does not. The risk of waiting is not only ongoing pain. It is that your body starts to protect the area by changing how you move, healing occurs under incorrect load/movements & protective mechanism and reflexes really lock in.
When sciatic pain hangs around, the body commonly adapts by shifting weight off one side (known as antalgia), shortening stride length (altered gait), bracing through the trunk, avoiding bending and rotating, as well as an overall desire & capability to move at all. Those compensations can keep the body stressed, the spinal nerve/s irritated and also spread stress into the hips, mid-back and the opposite side. Then even when symptoms start to calm, the movement pattern remains faulty. You capacity for activity and function is reduced. That is where recurring flare ups often come from; a never ending cycle of injury, recovery, re-injury.
Early assessment changes the trajectory. When we reduce irritation sooner, restore motion sooner and guide safer movement sooner, symptom improvement can translate into better long-term function. This is the key point.
Short-term symptom management is important, but the goal is bigger; DEEPER. We are aiming for greater long-term functional capacity and a lower likelihood of re-injury by interrupting the compensation cycle early and rebuilding strength and control consistently over time.

Why consistency matters: sciatica is rarely a one-visit fix
People often stop once the pain drops. A common thought proccess rooted in how we ‘FEEL’ is how we function. Wrong. That is understandable however, but it is also why sciatica keeps returning.
There are usually three phases:

Function: normalize movement and posture under daily loads through improved mechanics and control
